Collagen doesn’t just decrease in quantity; its quality also changes. While young skin has regular, tightly packed Type I and III collagen fibers, these fibers become irregular and fragmented with aging. Connections with elastin fibers weaken.

In advanced age (55-70 years), managing expectations correctly is important. In severe sagging, it may not be sufficient alone. I use it as combination treatments or pre-surgical preparation. It’s also ideal for post-surgical maintenance.

It’s safe in a wide range of skin types. Since it doesn’t target melanin, there’s no pigmentation risk. This is a big advantage especially for our dark-skinned patients.

Contraindications and Things to Consider

As with every treatment, there are situations where Sofwave is not suitable. Active skin infection and open wounds prevent treatment.

There’s no safety data during pregnancy and breastfeeding, so I don’t recommend it. Hormonal changes can also affect results. Treatment can be done 3 months after breastfeeding ends.

Autoimmune diseases, especially collagen vascular diseases (lupus, scleroderma) are contraindications. There’s risk of abnormal healing reaction in these diseases. Controlled diabetes is not a problem, but uncontrolled diabetes can impair healing.

Keloid history is a relative contraindication. The risk is low but still should be careful. I first try a small test area. Anticoagulant use is not a problem, bruising risk is already minimal.

If filler or botox has been done recently, wait 2 weeks. If Sofwave is done first, injection can be done immediately after. This order is important – in the reverse situation, the injected material may displace.

Combination Treatments: Synergistic Approach

I can use Sofwave as monotherapy, as well as include it in combined protocols. In the “total facial rejuvenation” concept, each treatment targets different aging signs.

Sofwave + Botulinum toxin combination is a protocol I frequently apply. First I provide tightening with Sofwave, then correct dynamic wrinkles with botox 2 weeks later. I see synergistic effect especially in the forehead and eye area.

Compared to thread treatments, Sofwave stands out for being non-invasive. Threads have risks like infection, asymmetry, thread migration. Sofwave doesn’t have these risks. However, threads don’t have the immediate lifting effect – patience is needed with Sofwave.

Sofwave treatment may seem costly at first glance. Single session prices show regional differences, but there’s a certain standard in quality clinics. However, when long-term cost-effectiveness analysis is done, the picture changes.

Calculate the luxury creams, serums you use daily, monthly treatments. Calculate the annual cost. Considering Sofwave’s effect lasts 12-18 months, you’ll see it’s actually more economical. Moreover, no cream can provide the results Sofwave offers.

Compared to surgical alternatives, Sofwave is much more cost-effective. You can get risk-free and painless results at 1/5-1/10 the cost of a facelift surgery. When we also account for the loss of work force during post-surgery recovery, the difference widens further.

Package programs and installment options make treatment accessible. Some clinics offer combination packages – Sofwave + Botox + Filler etc. These packages both provide cost advantage and increase treatment effectiveness.

Home Care and Lifestyle Recommendations

Post-Sofwave home care routine is critical for optimizing results. I recommend using antioxidant serum after morning cleansing. Vitamin C, vitamin E, ferulic acid combination is ideal. Peptide-containing eye cream supports eye area tightening.

Retinoid use supports collagen production. Can be started 1 week after Sofwave. Initially 2-3 times a week, can be used nightly as tolerated. Retinol 0.5-1% concentration is sufficient, if using tretinoin, 0.025-0.05% is appropriate.

Seasonal Protocol Recommendations

Special Cases and Protocols

  • Melasma patients: Sofwave doesn’t activate melasma but should be careful. I apply low energy, single pass protocol. Pre/post treatment tranexamic acid use provides safety.
  • Rosacea patients: Risk of activating vascular component is minimal but exists. I keep cooling at maximum and reduce energy to 80%. I add LED therapy post-procedure.
  • Acneic patients: If there’s active acne, it should be treated first. Sofwave can slightly reduce sebum production, this is an advantage. However, if there’s inflammation, should wait.
  • Thyroid patients: Controlled thyroid disease is not a problem. However, should be careful in neck area. I avoid direct application over thyroid.

It’s important not to fall into the overtreatment trap. Not every sagging is a Sofwave indication. Surgery should be recommended in severe cases. I believe in thinking about the patient’s benefit, not their money. Especially in a period when advertisements are so widespread, it’s very important to choose a physician who really thinks about you and can perform the right treatments.
